(hereinafter called “Seido Karate, Honbu”) agrees to allow the applicant and the applicant’s parent or guardian, if applicable, (hereinafter collectively “The Applicant”), to participate in Seido Karate Honbu’s Bring Your Parent to Class Day, taking place on May 30th, 2015 at 61 West 23rd street, 2nd Floor NY, NY 10010 (hereinafter called “The Event”) in consideration of, and in reliance upon, the execution of this Agreement.  The Applicant warrants, represents and acknowledges that Seido Karate Honbu has fully informed him or her of the nature and risks involved in the karate and athletic activities to take place at The Event conducted and taught by Seido Karate Honbu, that he or she understands said nature and risks, and that he or she is physically and mentally fit to participate in such activities. The Applicant has examined the physical premises, the supervision arrangements and all other aspects of the arrangements for The Event and has determined that Seido Karate Honbu has exercised due care, has been reasonably prudent, and has not been careless in any way as to any of the same. The Applicant agrees that, in further consideration, being permitted to participate in The Event, he or she assumes the risk or any and all accidents and injuries of any kind sustained by her or him by reason of or in connection with said karate and athletic activities at The Event or otherwise, whether such injury or accident be sustained at The Event or at any training location. The Applicant hereby releases, discharges and absolves Seido Karate Honbu and its agents and employees of any and all liability or responsibility for any accidents or injuries, whether the same are caused by or attributed to their negligence or the negligence of any of them.  The Applicant also agrees to assume the risk of any accident or injury of any kind sustained anyplace on the premises of Seido Karate Honbu including, but not limited to, the exercise area or anyplace in the building in which The Event takes place, or on entering or leaving such building, and hereby releases, discharges and absolves Seido Karate Honbu and its agents and employees of any and all liability or responsibility for any accidents or injuries, whether the same are result of the negligence or gross negligence of Seido Karate Honbu or otherwise. The Applicant undertakes that he or she will not use the knowledge and skills acquired from Seido Karate Honbu in any unlawful manner and further agrees that he or she will abide by the rules and regulations set forth by the Seido Karate Honbu.  The Applicant consents to the use of, and waives any compensation whatsoever for, all pictures, movies, tapes, TV, media coverage, etc. by Seido Karate Honbu or associated entities in the World Seido Karate Organization. The Applicant agrees that all covenants and agreements extending to and between Seido Karate Honbu also extend to and with the World Seido Karate Organization.  The terms and conditions set forth physically above this Agreement are made a part of this Agreement and incorporated herein.  I have read this Agreement and understand that once it is signed by me it is a legally binding and enforceable obligation and I agree to comply with all the provisions, terms and conditions set forth in this Agreement.
